So the problems seems to be on the TAPI side then.

Can you please download http://www.julmar.com/tapi/tapi_soft_phone.zip and try if you can place a call with the phone.exe utility.
1) Choose one of the DIVA Analog channels
2) Start session
3) enter a phone number
4) Dial

Sorry for the troubles, but without having access to your installation it is difficult to pinpoint the problems. Therefore we need to go though step-by-step.
